A Proud Achievement for Our Team: 1st Rank Pan India in Post Discharge Medication Adherence

in Colombia-Original • 25 days ago

Greetings of the day,

This is @alfazmalek from #India,

Today felt like one of those days that I would like to remember for a long time. Nothing about the day was planned as a special personal occasion, but somehow it turned into a beautiful combination of professional achievement, teamwork, gratitude and family time. Looking back at the day tonight, I feel that some days become memorable not because of one big event, but because of the small moments that come together and make the entire day meaningful.

One of the biggest highlights of the day was a proud moment at work. Our team at HCG Hospitals received recognition for achieving 1st Rank Pan India in Post Discharge Medication Adherence. Seeing the award and being part of this achievement gave me a different kind of satisfaction. Post discharge medication adherence is something that can easily be overlooked because once a patient leaves the hospital, it may feel like the treatment journey is almost complete. In reality, that is when another important part of patient care begins. Ensuring that a patient understands the prescribed medicines, knows how and when to take them, and continues the medication properly after going home can have a significant impact on recovery.

Today, while looking at the award, I was also thinking about the effort behind it. Achievements like this are rarely the result of one person's work. They are built through the contribution of many people, from healthcare professionals involved in patient counselling and education to those who support follow-up and ensure that the process continues smoothly. Medication adherence is not simply about giving medicines to a patient. It is about communication, understanding, trust and making sure that the patient and family are confident enough to continue the treatment at home. When these small things come together, they can make a real difference to patient outcomes. That is why this recognition feels more meaningful than just a rank or an award.

There was also something special about today's achievement because it reminded me why healthcare is much more than a job. We often spend our days discussing patients, processes, audits, targets, quality indicators and improvements. Sometimes the routine becomes so familiar that we forget the purpose behind all of it. But moments like today bring that purpose back into focus. Every improvement in a process ultimately has a patient and a family behind it. If better medication adherence can help a patient recover more effectively, avoid complications or continue treatment without interruption, then the effort is worth it. Being recognized for something connected so closely with patient care made today's professional achievement especially satisfying.

But after all the excitement and professional responsibilities, the evening became much simpler and, in its own way, even more special. I went out with my son, Uves, to have pizza. There was no award ceremony, no presentation, no professional discussion and no pressure to achieve anything. It was simply a father and son spending some time together over pizza. Watching him enjoy the food, seeing his expressions and just sitting there with him reminded me that happiness does not always need a big reason. Sometimes a simple meal with your child after a long day is enough to make you feel completely relaxed.

While sitting there, I found myself thinking about the contrast between the two parts of my day. During the day, I was celebrating a professional achievement that came from teamwork and dedication. In the evening, I was enjoying one of those ordinary family moments that may not look important from the outside but become precious memories over time. Both moments gave me happiness, but in completely different ways. One gave me a sense of accomplishment and pride, while the other gave me peace and a feeling of gratitude. Perhaps that is what makes life beautiful. We keep working toward bigger goals, but at the same time, the simplest moments often become the memories we hold closest to our hearts.

As I write this at the end of the day, I feel grateful for everything that happened today. Grateful for the team, grateful for the recognition, grateful for the opportunity to contribute to better patient care, and most importantly, grateful for the people who make life meaningful beyond work. Today was not just about receiving an award or achieving the 1st Rank Pan India. It was about remembering why the work matters and then coming home to enjoy a simple evening with my son. Some days give us achievements to celebrate, and some give us memories to cherish. Today gave me both. And perhaps that is what made it a day worth writing about in my diary.
