A stroke (cerebrovascular accident) is a medical emergency that occurs when a blood vessel in the brain becomes blocked or ruptures; according to data from the World Health Organization (WHO), it represents one of the greatest global health burdens. A stroke cuts off the supply of oxygen and nutrients to brain tissue, causing brain cells to begin dying within minutes. There are two main types: ischemic, when a clot blocks an artery in the brain, and hemorrhagic, when a blood vessel ruptures and bleeds into or around the brain.

Un accidente cerebrovascular (ACV) es una emergencia médica que ocurre cuando se bloquea o se rompe un vaso sanguíneo en el cerebro, según los datos de la Organización Mundial de la Salud (OMS), representa una de las mayores cargas de salud a nivel global. The latest data from the WHO reveal the significant global impact of this disease. Approximately 11.9 million new cases of stroke occur worldwide each year. It is estimated that nearly 93.8 million people live with the aftereffects of a stroke, making it the third leading cause of death and disability combined on a global scale. Fortunately, it appears that Duke University in the United States has developed a treatment that can help the brain regenerate.

Los datos más recientes de la OMS muestran el gran impacto de esta enfermedad en el planeta. Cada año se producen unos 11,9 millones de casos nuevos de ACV en el mundo. The treatment consists of an injectable biomaterial designed not to regenerate neurons directly, but to create a physical and biological environment that enables the body itself to repair damaged brain tissue following an ischemic stroke. The system combines two main components: a physical scaffold made of hydrogel microparticles (known as MAPS—Microporous Annealed Particle Scaffolds) that, when injected as a liquid, assemble to form a porous, sponge-like structure within the cavity left by the stroke. This structure serves as a three-dimensional support, allowing the body's cells to migrate, adhere, and rebuild tissue.

El tratamiento consiste en un biomaterial inyectable diseñado no para regenerar neuronas directamente, sino para crear un entorno físico y biológico que permita al propio organismo reparar el tejido cerebral dañado tras un accidente cerebrovascular isquémico. El sistema combina dos componentes principales, un andamiaje físico formado por micropartículas de hidrogel (llamadas MAPS, por Microporous Annealed Particle Scaffolds) que, al inyectarse como líquido, se ensamblan formando una especie de esponja porosa dentro de la cavidad que deja el ictus. Esta estructura sirve como soporte tridimensional para que las células del cuerpo puedan migrar, adherirse y reconstruir tejido.

Researchers extract extracellular vesicles (EVs) from astrocytes—brain support cells—and chemically anchor them to hydrogel microparticles. Inside these vesicles, they incorporate specific signaling molecules, such as IL-4 and C1q, which act as a "lure" to attract beneficial immune cells to the injury site. The key finding is that these signals function only when anchored to the scaffold; if the vesicles are injected in a free-floating state, they disperse and fail to achieve the reparative effect, demonstrating that the physical structure is essential for localizing and coordinating the response.

The potential impact of this technology is significant because it addresses a problem that current treatments do not solve. Emergency treatments (thrombolysis or thrombectomy) restore blood flow but cannot replace brain tissue that has already died. Following a severe stroke, a cavity remains that rehabilitation alone cannot rebuild. In mice, the treatment promoted the growth of new blood vessels and nerve fibers within and around the cavity. Most notably, in motor coordination tests, treated mice matched the performance of healthy mice by the eight-week mark, and the improvement was sustained.

El impacto potencial de esta tecnología es significativo porque aborda un problema que los tratamientos actuales no resuelven. The biomaterial was injected more than 24 hours after the stroke—a point at which current reperfusion therapies are no longer effective. This opens up a therapeutic window for patients who arrive late at the hospital. However, as always, this is preclinical research involving mice; further steps are required before it can be applied to humans. The study utilized vesicles derived from rat astrocytes. The next step is to generate them using human pluripotent stem cells, an approach that would be more scalable and clinically relevant. Safety must be assessed, the treatment tested in larger, more representative animal models, and the findings confirmed in the context of human stroke.

El biomaterial se inyectó más de 24 horas después del ictus, un momento en el que las terapias actuales de reperfusión ya no son eficaces. Esto abre una ventana terapéutica para pacientes que llegan tarde al hospital. Pero, como siempre, esto es investigación preclínica en ratones. Aún quedan pasos antes de que pueda aplicarse en humanos. El estudio usó vesículas de astrocitos de rata. El siguiente paso es usar células madre pluripotentes humanas para obtenerlas, lo que sería más escalable y relevante clínicamente. Existen tantas personas afectadas por un ACV que este tratamiento se perfila como una luz al final del túnel. Esperamos que puedan llevarse más pruebas para ser usado clínicamente con toda la seguridad en los seres humanos.
